Chronic Disease Management Program

A chronic disease refers to a long-term illness, and most of them can persist throughout a person's life.

Under the Singapore government's initiative, the Chronic Disease Management Programme (CDMP) was introduced by the Ministry of Health (MOH) on 1st January 2021 with aims to improve the level of care and reduce the healthcare cost for individuals with chronic diseases when they seek medical treatment as an outpatient.

The following are the 20 chronic diseases listed by MOH:

  • Anxiety

  • Asthma

  • Benign Prostatic Hyperplasia

  • Bipolar Disorder

  • Chronic Kidney Disease (Nephrosis/Nephritis)

  • Chronic Obstructive Pulmonary Disease (COPD)

  • Dementia

  • Diabetes Mellitus / Pre-diabetes

  • Epilepsy

  • Hyperlipidemia (Lipid Disorders)

  • Hypertension

  • Ischaemic Heart Disease

  • Major Depression

  • Osteoporosis

  • Osteoarthritis

  • Parkinson's Disease

  • Psoriasis

  • Rheumatoid Arthritis

  • Schizophrenia

  • Stroke

 

MediSave can now be used at Public Hospital Specialist Outpatient Clinics (SOC), polyclinics, and participating clinics for partial outpatient medical costs for the above mentioned 20 chronic conditions.

Each patient with complex chronic conditions can use up to S$700 annually. The entitlement for other patients will be S$500 maximum per patient yearly for their treatments. Each MediSave claim is subject to a 15% co-payment in cash.

Community Health Assist Scheme (CHAS) cardholders with chronic conditions can also draw on the government subsidies up to S$500 per year, while Pioneer Generation or Merdeka Generation cardholder special subsidies are capped at S$540 per year.
